The video above demonstrates what the rail car manufacturer says is the sequence of events leading to the failure of rail car axles and a derailment of the LRT light rail cars on Ottawa’s system.

The Ottawa Rail Transit Group and the manufacturer still haven’t agreed on the root causes of these failures or the corrective action needed. However, at a meeting about the problems, an RTG representative said they were within weeks of agreeing on the root causes.
